ASSISTED’S ENHANCED HOME HEALTH PROGRAM

Assisted Home Health is one of only a handful of agencies in Southern California to offer the ground breaking program know as Enhanced Home Health. The purpose of the Enhanced Home Health program is to reduce emergencies and readmissions with frequent patient communication and help coordinating patient care.

Enhanced Home Health Features:

• Pre-discharge visit from onsite home health liaison
• Discuss and answer questions about services that the agency will provide
• Weekday and weekend visits
• Tuck in calls that identify red flags
• Frequent communication to reduce avoidable readmissions
• Improve patient satisfaction and compliance

Patients involved in the program have seen readmission rates reduced by up to 32%. Our goal is to ensure patients receive excellent care and to reduce hospital readmissions.
