As a LVN Intake/Staffing Coordinator you will:

  • Takes and prepares non-clinical components of intake referrals.
  • Follows up on entry and distribution of referrals.
  • Assists intake team in phone coverage and coordination of day-to-day activities.
  • Participates in team and goals of the Agency.
  • Verifies insurance coverage for services, if needed.
  • Provides clinical, billing, and collections personnel with information regarding insurance coverage.
  • Tracks authorization versus utilization and secures authorization for visits.
  • When requested, provides report to case managers of third party payors after clinical review.
  • Perform data entry functions for service-related charges.
  • Acknowledges patient rights including privacy and confidentiality.
  • Consistently displays a professional appearance and conduct in all settings. Customer service, including interpersonal and interagency relationships, is positive and professional at all times.

Staffing Coordinator

  • Schedules all shift requests for Home Health. Shifts may require RN’s, LVN’s, have variable start and stop times, and are a minimum of four hours in length.
  • Creates 3-ring binder charts for staff use in patients’ home as needed.
  • Acts as chief liaison to patient and family regarding personnel and shift issues.
  • Provides additional copies of face sheet, patient address map, and staff schedule for each staff member for patient’s home as needed.
  • Obtains staff availability and private duty shift needs on an on-going basis for the upcoming week.
  • Qualifications:

    • Graduate of accredited nursing program. Current, active license as an LVN by the California Board of Nursing including fingerprint card.
    • Current CPR, health clearance, TB clearance.
    • Successful passage (80%) of Agency competency exam, clinical skills self-assessment and core competency skills checklist.
    • Minimum of one year experience within the last three years.

    Must have visual and hearing acuity. Must be able to communicate clearly verbally. Must be able to read, write and speak English fluently. May be exposed to minor hazards/communicable diseases. Physically active walking, standing, bending, lifting, and climbing stairs. Must be able to perform fully assisted transfers, with or without transfer devices (if required for the position). Must not have any back lifting/moving restrictions. Must provide proof of automobile insurance upon hire and at each renewal and maintain a safe driving record.
