Occupational Therapists through Assisted Resident Wellness Program will help assess ADL performance, make recommendations to maximize independence and help seniors overcome areas of difficulty.

Fall prevention goes a long way towards keeping residents in a community where they are comfortable and independent. Seniors with decreased strength or poor balance can benefit from working with physical therapists to prevent falls. According to the CDC, falling causes 800,000 hospitalizations and 95% of all hip fractures. These type of injuries and fractures frequently result in a move to a skilled nursing facility. Physical Therapists through Assisted’s Resident Wellness Program can assess for fall risks and structure an exercise plan to help residents build strength and balance.

Nursing Services & Coordination of Care

The Assisted RWP team works hand in hand with physicians to coordinate care as well as identify signs and symptoms of change in condition. Our nurses instruct in the administration of medications to control symptoms and help to avoid hospitalizations. Assisted’s Registered Nurses and Licensed Vocational Nurses provide the needed support and monitoring that can improve the lives of the residents.
