Who can join GUIDE?

  • Have Medicare Parts A & B as a primary payor
  • Have a documented dementia diagnosis
  • Not reside in a skilled nursing facility or receive hospice care
    • Infographic outlining eligibility for GUIDE services: patients with dementia can receive support from the Caregiver Guide Program if they have Medicare Part A and B; not eligible if on Medicare Advantage, in hospice, or in a skilled nursing facility.

    How does GUIDE work?

    Care Navigation & Planning:

    A Care Navigator stays in touch with regular check-ins and helps develop a personalized care plan.

    Community & HomeBased Services:

    We connect you to community programs and arrange home safety visits when appropriate.

    Caregiver Respite & Relief:

    We help arrange respite care and provide guidance when caregivers need a break.
